The Employment and Community First CHOICES program, administered by TennCare through its contracted managed care organizations, focuses on providing services to people with intellectual and developmental disabilities (I/DD). Through the services a person with I/DD can explore work possibilities and be provided the supports needed to learn new skills for work, find a job, and keep a job. Employment may vary from part-time to full time or even self-employment. Other services include community Living Support (CLS).

We provide supported employment services designed to assist persons with severe or significant disabilities in obtaining and maintaining community and integrated competitive employment through specifically planned supports.
Supported Employment services consist of ongoing supports designed to assist individuals for whom it is unlikely that competitive employment at or above the minimum wage will occur without supports, and who, because of their disabilities, need supports to perform in a regular work setting.
The planned outcomes of these services are increased hours worked, increased wages and frequent opportunities to interact with non-disabled peers during the normal performance of the job and/or during breaks, lunch periods or travel to and from work.
This service provides people with disabilities the opportunity to learn what employment can look like and decides if they would like to pursue integrated employment or not
This service is offered to persons with disabilities who wish to pursue individualized integrated employment or self-employment but for whom more information is needed to determine their strongest interests, Skills, and conditions necessary for successful employment or self-employment
This service is provided to persons with disabilities who are interested in working and have multiple areas of interest. The service provides them with the opportunity to experience selected job choices and narrow down to the one they like best. The service involves observation and assessment of an individual’s interpersonal skills, work habits and vocational skills through practical experiential, community integrated volunteer experiences and/or paid individualized, integrated work experiences that are uniquely arranged and specifically related to the interests, preferences and transferable skills of the job seeker as established through Discovery or a similar process.
This service is provided to persons with disabilities to teach “non-job-task-specific” strengths and skills that will help get a job such as communication skills, workplace conduct, professional dress code, job interviews practice, job search and applications, following directions, safety, professional ethical behavior etc.
Job Development is a services provided by employment specialist to persons with disabilities who are ready to work, but need professional assistance in collecting existing functional data needed by employers or self-employment start-up.
This service is provided to persons with disabilities with the target to create a clear and detailed plan for Job Development or for the start-up phase of Self-Employment.
The Job Development service provides the supported person with disabilities the opportunity to obtain a competitive or customized job in an integrated employment setting in the general workforce, for which the individual is compensated at or above the minimum wage, but ideally not less than the customary wage and level of benefits paid by the employer for the same or similar work performed by individuals without disabilities.
Job Coaching for Individualized , Integrated Employment includes identifying, through job analysis, and providing services and supports that assist the individual in maintaining individualized integrated employment that pays at least minimum wage but ideally not less than the customary wage and level of benefits paid by the employer for the same or similar work performed by individuals without disabilities This service is provided to persons with disabilities who have successfully acquired an employment opportunity. An employment consultant/job coach is assigned to assist the employed person with learning new skills and forming co-worker relationships. The job coach works closely with both employer and the employed person to identify what new skills and behaviors are to be learned and the process and standards by which performance will be evaluated.
career planning and advancement support service for persons currently engaged in individualized integrated employment or self-employment who wish to obtain a promotion and/or a second individualized integrated employment or self-employment opportunity. The service is time-limited and focuses on developing and successfully implementing a plan for achieving increased income and economic self-sufficiency through promotion to higher paying position or through a second individualized integrated employment or self-employment opportunity.
The outcomes of this service are:
It provides Social Security disability benefits (beneficiaries).
Provides counseling, guidance, training, job placement, and services to support retention.
Provide Attendant Care, Personal Care, Respite Care, and Community Living Support Homes.
Support for conditions like Alzheimer’s, Diabetes, Parkinson’s, Dementia, etc.,
Provide Support and Care to clients discharged from acute healthcare settings and injuries.
