We Accomplish this Mission by Partnering with MCOs, SSA, and VR to support people with disabilities integrate into the community and maintain supported Employment. Our Ticket To Work program provides Nation Wide Employment Network (EN) services.

Services/what we do

We provide a wide range of services tailored to help people with disabilities live purposeful and fulfilling lives in the community. We are licensed by the TDMHSA and the DIDD.
We partner with MCOs, SSA, and VR to provide Community living support and Supported Employment Services.
We also work with privately paid and other forms of service reimbursement payments.

ECF CHOICES

Support for people of all ages with an intellectual or developmental disability (I/DD).

Ticket to work

It provides Social Security disability benefits (beneficiaries).

Vocational Rehabilitation

Provides counseling, guidance, training, job placement, and services to support retention.

Senior Care

Provide Attendant Care, Personal Care, Respite Care, and Community Living Support Homes.

memory Care

Support for conditions like Alzheimer’s, Diabetes, Parkinson’s, Dementia, etc.,

Post-Acute Care

Provide Support and Care to clients discharged from acute healthcare settings and injuries.

Covid-19 has imprisoned our mother in her assisted living facility for over six months, as per the TN Governor’s mandate. Home Care Preference Caregiver can keep our mother “afloat” during this challenging time. Many days they do arts & crafts or bake. These activities make our mother feel Capable as many of the Organized activities at her Assisted living residence are too challenging for her mentally. We are so grateful to supplement the support she needs, hopefully, keep her healthy through this pandemic, and work with such a caring organization.
